All HMO Plans require a referral with a Referral Authorization Number.
Please, verify with your insurance whether a referral is required. If we are not in the network with your insurance, or you don’t have a health insurance coverage, we will be happy to see you as a self-pay patient.
The fee for the new self-pay patient visit/consultation/procedures is between $100 and $250 depending on the service provided.
